Investigation enquiry form

The purpose of this form is for local support to be arranged for RCN members who are being formally investigated at work.

If you are not the subject of a new investigation or disciplinary, or you want to speak to someone, this form is not right for you. Please visit our help pages where you can read our online advice and find alternative ways to contact us.

If you are a witness and your practice is not being brought into question, you do not need to complete this form. You can find further information on being a witness in our witness advice guide.

It is not usually necessary to have a representative at an investigation or fact finding meeting. In fact, there is no legal right to representation, and many employers won’t allow a representative to be present, or to speak if they are allowed to attend. However, it is important that we properly assess so please complete the below form.

If you have been invited to a disciplinary meeting, or you have been asked to write a statement regarding your practice, please complete the below form.

As you will not be speaking with us, it is important that you have read our guidance, and that you complete all sections in full. This will enable us to assess your situation and refer you for support without delay.

Please note that RCN Direct will not usually call you. Where an onward referral is required, provided you were in the correct category of membership at the time of the incident, we will provide these contact details to your local regional/country office.

Investigation or disciplinary

This is the date when an event occurred that led to the action being taken. It is not the date you were told about the meeting or received a letter. You must have been in the correct category of membership on this date to receive support.

If you have been suspended from work, remember that suspension is ordinarily considered a neutral act, and does not imply guilt.

Whilst we don’t require in-depth detail at this stage, you must give us enough information so that we do not need to contact you for further information, as this may cause unnecessary delay in arranging support.

Please note, we do not need to see the correspondence between yourself and anyone else involved at this stage; however, you should retain all documentation as your local RCN team may require this.
